Client:

Hawaii Med-QUEST

Challenge:

Develop requirements for Hawaii’s Medicaid Managed Care Organizations (MCOs) to screen and intervene on social risk factors for high cost / high needs Medicaid members.

Response:

Bailit Health’s technical assistance focused on two major tasks: to prioritize the social risk factors Hawaii’s Medicaid MCOs would screen for; and to select a validated social risk factor screening tool for all MCOs to use to screen high cost / high needs members for the prioritized social risk factors.

Client:

San Bernardino County

Challenge:

Establish a collaboration between Arrowhead Regional Medical Center (ARMC) and County Department of Behavioral Health (DBH) and Department of Public Health (DPH) leadership, along and with key stakeholders, to collaboratively design and implement a population health management strategy and work plan. The strategy and plan spanned across physical, behavioral, and social health domains to improve outcomes, reduce disparities, and position San Bernardino County as a high-value integrated public health care system.

Response:

Bailit Health is supporting San Bernardino County to develop its population health management strategy. Key activities included engaging executive leadership of the County health system, conducting stakeholder interviews, developing the strategy, and facilitating five work groups. The work groups focused on program and resource alignment, behavioral health integration and primary care, social determinants of health, health equity, and community engagement, value-based purchasing infrastructure and models, and data analytics and IT.

Client:

Arizona Health Care Cost Containment System (AHCCCS)

Challenge:

Assist AHCCCS in developing provider requirements for SDOH and health equity within its 1115 Research and Demonstration Waiver renewal.

Response:

Bailit Health provided technical assistance to AHCCCS on key policy decisions to help create provider requirements related to SDOH and health equity, such as to direct providers to develop workflows that govern how SDOH screening and intervention is performed and to stratify screening results by race, ethnicity, and language to identify health disparities.

Client:

Rhode Island Executive Office of Health and Human Services (EOHHS)

Challenge:

To develop a quality measure for use in contracts between Medicaid managed care plans and Accountable Entities (Medicaid ACOs) in Rhode Island focused on screening individuals for social risk factors.

Response:

Bailit Health convened a Work Group of payers, providers and state agency representatives to identify priority SDOH domains, identify the measure population, develop measure specifications, and implement the measure in EOHHS’ Accountable Entity quality incentive program.
